In addition to the types of Personal Data listed in the Privacy Policy, the following additional Personal Data about you will be collected in order for us to provide you with WLC Services: 

• Home address/shipping address
• Payment information
• Emergency contact/next of kin information including name, phone number and relationship 

Sensitive Data

In addition to the types of Sensitive Data listed in the Privacy Policy, the following additional sensitive data about you will be collected in order for us to provide you with WLC Services:

• Cultural/ethnic background
• Pregnancy status
• Physical activity levels, primary health goals and target weight 
• Weight loss method history and food consumption behaviour 
• Current and prior medical conditions including any allergies
• Current medications and current supplements 
• Primary General Practitioner name and place of practice 
• Mental health and wellness history 
• Relevant previous medical history including immunisation record, risk factors and adverse health events
• Alcohol consumption frequency 
• Smoking history
• Health identifier and/or Medicare number 
• Health information including medical consultation notes, health summaries and reports, test results and other reporting correspondence (including referrals).

By engaging fatsecret to provide you with any of the WLC Services or providing us with any of the above Sensitive Data you consent to our collection, use and disclosure of your Sensitive Data detailed in this Collection Notice and the Privacy Policy. You have the right to withdraw your consent to our processing of your Sensitive Data at any time. However, if you do so (or if you do not provide us with the requested Sensitive Data in the first place) we will be unable to provide you with some (and possibly all) of the WLC Services. 

Personal Data

In addition to the uses listed in the Privacy Policy, Personal Data about you collected for any of the WLC Services will be used for the following purposes:

• To facilitate the delivery of prescription weight loss medications to your home or designated shipping address
• To process payment for WLC Services
• To communicate with you in relation to WLC Services
• To otherwise facilitate the provision of WLC Services
• As permitted or required by law

Sensitive Data

In addition to the uses listed in the Privacy Policy and above for Personal Data, Sensitive Data about you collected for any of the WLC Services will also be used for the following purposes: 

• To assess your eligibility for the medicated weight loss program
• To assist medical practitioners in providing you with medical advice or care, including to provide referrals or pathology testing orders
• To otherwise facilitate the provision of WLC Services
• As permitted or required by law

Personal Data and Sensitive Data

In addition to the disclosures listed in the Privacy Policy, Personal Data about you collected for any of the WLC Services will also be disclosed as follows to: 

• Medical practitioners
• Other health professionals including dietitians and nutritionists
• Pathology providers or other health providers
• Pharmacy providers
• Courier services
• MediRecords, Coviu, DialPad
• Other third party service providers
• Australian Government Departments if you request this  
• As permitted or required by law

As noted above, fatsecret engages third party service providers to support WLC operations and to provide our services. This may mean that for the purposes noted in this Collection Notice your Personal Data may be shared with those service providers, some of whom may be located overseas.

Your Sensitive Data will only be shared with third party service providers whose processing you have consented to via this Collection Notice or the fatsecret Privacy Policy.
